What we are talking about
Cerebral palsy, spina bifida, spinal cord injury, neuromuscular diseases, Down syndrome, autism with continence difficulties: the causes differ, the needs are alike. A teenager or young adult no longer fits children's sizes and not yet the usual adult sizes, spends long hours seated, and cares about privacy even when unable to say so.
Here we talk products, routine and respect. Medical follow-up, bladder training and any catheterisation remain the care team's business.
Size first
Most brands jump from junior nappies to an adult M. Nateen has a specific line for teenagers, between baby and adult: the Combi XS “Youth / Junior”, a brief with tabs for a 55 to 85 cm waist (21.5 to 33 in) and 25 to 40 kg, Plus level, in packs of 10. When it becomes too small, the Combi S takes over, for 65 to 105 cm and 35 to 55 kg. M starts at a 95 cm waist and 50 kg. Measure the waist at the navel and the hips at the widest point; between two sizes, take the smaller if the thighs are slim, the larger if the belly is round.
A product too large gapes at the groin and leaks as soon as the young person sits; too small, it marks the skin and overflows at the thighs. That is the first cause of leaks in young people, before absorbency.
Three situations, three ways
School, outings, sport: if standing is possible, even with support, Flexi pants go on and off like underwear, alone or with help, without lying down. If the change happens after a transfer onto a table or bed, the Combi brief with tabs is quicker and closes without lifting the pelvis.
Wheelchair: seated for hours, the body's weight presses the absorbent core, which must hold the liquid instead of releasing it back to the skin. That is what rewet measures, explained in the absorption article, and why we publish that value. Change at the latest every four hours, and look every evening at the sacrum and sitting bones: redness that does not blanch under the finger is reported to the nurse.
At night: a Combi Ultra fitted higher at the back, because lying down sends liquid backwards, with a Mati underpad tucked under the hips. One check during the night, without bright light, and a full change in the morning.

Respect and autonomy
Involve the young person in every step they can do: choose the product, hold a tab, say when it is damp, put away the spare. Use neutral words, “protection”, “change”, never “baby nappy”, including between adults in front of them. A discreet pouch in the bag, with one spare, a wipe and a sealed sachet, saves depending on someone for everything.
A fixed routine reassures and prevents oversights: same times, same order, same place, at school as at home. The occupational therapist or school nurse help set it up and have every helper follow it.
The skin of a growing body
Growth spurts, sweat, wheelchair friction: a teenager's skin irritates quickly under a product. Washing with warm water or a Nateen wipe with a pH close to the skin's, patting dry, a thin layer of barrier cream on exposed areas, and no talc. Periods in girls are managed with the usual product, changed more often, or Flexi pants that absorb both.
